ABOUT THE ROLE

Reporting to the Clinical Manager –Geriatric Assessment and Rehabilitative Care Program, the Client Transition Coordinator is responsible for helping the organization further its mission by supporting transitions in care for older adults with a focus on early identification and prevention of frailty, restoration and rehabilitation, and health teaching; and remote care monitoring.  By providing support for geriatric clients from admission to discharge and 30 days post-discharge, it is expected frail older adults will have increased ability to stay safe and well in the community and thus reduce Emergency Department visits, hospitalization, ALC bed utilization and delay admission to Long-Term Care.  Key responsibilities include early identification, screening, assessing, navigating, and supporting clients eligible for the Geriatric Assessment and Rehabilitative Care program including Remote Client Monitoring (RCM) program.

WHAT YOU BRING

 

Education/Experience:

  • Current unrestricted certificate of registration with the Ontario College of Nurses
  • Current BLS certification
  • Experience supporting clients through Ontario Telemedicine Network
  • Previous experience related to Remote Client Monitoring
  • Minimum of three years’ experience providing health care services to the geriatric population
  • Advanced knowledge and understanding of best practices related to dementia, delirium, mental health and its effect on the person and family members/care partners

 

Skills/Abilities:

  • Superior ability to communicate effectively and concisely, both orally and in writing and through a variety of means
  • Effective listening, observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation
  • Ability to independently plan and balance diverse and conflicting workload pressures, in a timely manner accomplishing program goals while meeting client needs
  • Ability to participate/lead quality improvement initiatives
  • Demonstrated project management skills and with proven experience in effective team leadership with the demonstrated ability to organize, prioritize and multitask efficiently
  • Strong interpersonal skills with the ability to work with diverse client groups with varying levels of comprehension and language capabilities
  • Committed to continuing education and self-directed learning with the willingness to participate in education offered by Ontario Health
  • Ability to function and contribute as an effective team member and to work independently
  • Ability to use independent judgment and maintain confidentiality of records, actions, and information
  • Proficient in computer database, spreadsheet and software applications
  • Excellent critical thinking, interpersonal, diplomatic and communication skills (verbal and written)
  • Excellent problem recognition skills with the ability to proactively achieve resolutions both through direction and negotiation and make decisions rapidly
  • Demonstrated knowledge, understanding, and commitment to client-centred care
  • Strong knowledge of community and regional services
  • Excellent computer, presentation and educational skills
  • Demonstrated regular attendance at work

 

Assets:

  • CNA Certificate in Geriatric Nursing
  • Membership in professional association e.g. RNAO
  • Experience working with Indigenous persons and First Nations
  • Knowledge of multi-cultural beliefs, values and perspectives with particular emphasis on Indigenous persons and First Nations

 

Conditions of Employment:

  • This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
  • Offers of employment made to external candidates are conditional upon providing proof of a complete primary series of Health Canada approved mRNA COVID-19 vaccine or proof of valid medical exemption, prior to the offered effective date.
